Make Music: Young People Acceptance Page 23/24

To accept your place on Make Music, Young People for 23/24 please follow the steps below by Monday 10th July.

1. Read the info:

At the bottom of this page are some links to pdfs. These include all the details for schedules and fees for all the different programmes, and our Fair & Square document to help everyone get the most out of their time with us.

2. Fill in the Acceptance Form:

Click here to complete the MMYP Acceptance Form. You will need your UNIQUE CODE from your offer email.

You should do one form for each young person in your care that’s joining the programme. This form gives us more details that we need to hold, and also includes consent questions, where we ask for your permission for your child to take part in the programme, for data usage, as well as asking you to confirm you understand our various terms and conditions. It must be completed by the parent/carer or a student over the age of 18.

3. Apply for Financial Assistance:

There are two bursaries available:

Music and Dance Scheme (MDS) You will need to download the forms below and photograph/scan them along with any evidence and email it to YMPbursary@sagegateshead.com, or you can post us a copy to:
